                       SECTION A: EDITORIAL

                    KILL ME - I'M USEFUL!
        - RECONSTRUCTING THE REAL WORLD IN CYBERSPACE -

Wherever we are, wherever we go, whatever we do - people build
on past experience and attempt to impose old frames of
reference to unstructured, novel situations, and high-
uncertainty conditions. The information superhighway is no
exception. Individuals naturally apply tactics which have
proved to be successful in the "real world" to the "virtual
world" as well. However, there are potential pitfalls to such
an approach, which the following explanations and examples
ought to make clear.

Cyberspace started a tabula rasa, history-wise first colonized
by technicians, hackers and eccentrics who probed its
possibilities and challenged its boundaries. Today, to the
extent that these boundaries exist, it is because we have
imposed them on ourselves. The Internet has come to be seen as
a "tool", a way to accelerate knowledge distribution, an
instrument of information acquisition and, most
controversially, a method of expansion and proliferation of
standards and structure, regulation, mainstream beliefs. In
essence, many people seem to think of the information super
highway as a fancy multi-membered high-tech fax machine.
Debates among InterPsych(TM) list members and coordinators are
illustrative of the phenomenon. They include increasing mail
volume, attending to all member questions, building new
boards, dealing with junk mail, requiring credentials for list
membership, and banishment from lists. All of these issues
show potential of being dealt with in one of the long-standing
ways of the physical world. The following discussion aims to
prove that the real-world methods are clearly worth
rethinking. Although examples may read challenging or - worse
- antagonistic, consider them as constructive comments by
stretching the arguments to the extremes.  Also note: All
quotes are authentic.

Example no. 1: There has recently been a discussion on
increasing the quality and utility, collegiality and user
satisfaction, or increasing "mail traffic volume", which is
low on some lists. There are no deus-ex-machina solutions to
these problems. Still, there are honorable, promising and
creative efforts to build and maintain group identity, serve
the customer by compliantly answering questions, creatively
organizing discussions, and approaching other areas of
concern.

One discussion item in this context has been "questions left
unanswered" by frustrated list members posting to a forum.
However, discussion forums are no Q-A vending machines. One
proposal has been to require that each message sent be
answered by members or by the listowner. However, in effect,
one does not know what to get, after input, which seems
disturbing to some individuals with a strong desire for clear
input-output contingencies as in computer processing. This may
urge them to impose mainstream beliefs and standard
procedures, which have demonstrated utility in the "real
world". But can you equate the "real world" with the "virtual
world"? Yes, if you view the former as a mere extension to the
latter. No, if you judge the Internet as a medium offering a
whole array of new possibilities, possibly unknown and
potentially unworkable in the "real world". Would you want to
miss out on these possibilities?

Example no. 2: Keyphrase "Trust the experts". A solution has
been proposed for improved management of lists, namely
building advisory boards in the background, and "review bodies
of any disputes". If you are at a loss in any matter, just
resort to those assemblies. Quote: "I believe that this
discussion has likely gone as far as it will go here, and the
issue will need to be taken up by the group W and board X".
Or: "Group Y [is] working on fixing up InterPsych forums. "
Or: "I am glad to hear group Z is working on the tension
between lists [...]". The point in question: Boards function
well in the hierarchically structured "real world". On the
egalitarian net, they create potentials of censorship and,
inversely, a deceptive comfort of always keeping the ball
rolling by the leaders. We're also making concessions to
democracy: Paradise lost in the only realm that was and still
could be the only place where individuals talk to each other
on equal terms - devoid of status, roles and official
functions.

Example no. 3: Keyphrase "junk mail". While, on one side,
complaints arise about low mail traffic, we also know of the
opposite phenomenon: Information overload and using the list
for "getting it off the chest". So, should we break up lists
into smaller sub-lists, eliminate "unsuccessful ones" or merge
them into bigger lists?

Employing the language of signal-to-noise ratios, think about
what's noise. The signal of one may be noise to the other. So,
even if threatened by statements like "without a reputation
for high level scholarly discussion, the best minds will
withdraw from InterPsych", who are we to judge, who the best
minds are or will be or will be not or will be by virtue of
regulation or owing to creating, tolerating and attending to
so-called noise? And what's high-level discussion? While
discussion of the Rorschach, MMPI, qualitative research
methods or psychoanalysis maybe viewed as lowly by some, they
may be judged entirely appropriate by others.

So, "low-level" could lead to "public psycho-chat" and we
don't want that, we, the InterPsych consortium, want to be "a
professional service". Quote: "[Although all lists should have
been open to] academics, clinicians, patients and relatives [...],
this just isn't going to work because the networks get
overloaded with trivia."  Drawing on analogues, HAM folks have
no problem with that: Tune in the channels you want, screen
out the noise, switch as you please. (But don't ask about the
acronym HAM; that's the "trivia").

Here's a real case: While there were already clinical-
psychology-related lists in existence with high-quantity and
lively discussion, they were judged as low-quality and
non-scholarly by renown clinical psychologists. Now we have a
separate list that aims to maintain that super-quality level;
result: lowest mail traffic as compared to the existing lists,
possibly striving for (quote) "journal-ready articulation of
ideas". Quote: "The 'hard' truth is that if we don't enforce
reasonable and well thought out exclusion rules there will be
no groups!" We have living evidence of that in the
here-and-now: A virtual no-list, officially sponsored, with
highest quality. Well-intended solutions that worked in the
real world do not necessarily work the same way on the net.

Example no. 4: Another related controversy has revolved around
exclusion-inclusion and the banishment of members by
non-compliance to rules. Quote: "Banishment and control are
natural properties of well-functioning groups. They happen
naturally. Forcing them may screen out people who: a) would
learn a lot from us; and b) would perhaps eventually teach us
something. Forcing them may also toxify and deaden the group
atmosphere."  Another quote: "The real question is how to
allow discourse, learning and community for all effected
groups. That's the challenge here."  If you have not been
affected yet, be aware that membership restrictions have
already been put into effect in some discussion forums. The
apparent paradox is that only the ones with demonstrable
credentials should participate, while also establishing
rules for lots of exceptions - and one time, we may have more
members-by-exception than regular ones...

Quote: "If Inter[P]sych is to flourish it must serve the
scholarly community, not the general public." Question:
"Doesn't the scholarly community serve the general public?"
Another quote: "I don't think that there is anything at all
[...] censorial in InterPsych trying to meet the needs of any
particular constituency". But which constituency do we
address? Or as another member has put it: "The real study of
the psyche of humanity occurs daily in the actions,
interactions and reactions of that general public which
Interpsych will not serve."

Scholarship is not a matter of credentials. It is a matter of
personal dispositions, enthusiasm, knowledge, experience and
interest in a subject matter and we don't want to drain
budding scholarship by rigid requirements for credentials,
so-called high quality input or structures that don't fit the
highly varying needs of groups and individuals. Of course,
there is a definite need for requirements, but there are
two sides of the coin. Internetting gives us the unique chance
to keep the undesirable side-effects of requirements and
regulations at minimum levels.

Example no. 5: Keyword "list moderation". Of course, we also
have scapegoats for list failure and membership
dissatisfaction: List moderators and counterproductive or
passive members. Sticking to the former: List moderators don't
react to questions quickly enough, they don't restrict discussions
carried on ad infitium and ad nauseum, they are too rigid or
too tolerant, and they don't fill the void if discussion on
the forum is sluggish. They are asked to indulge in activism
by acknowledging each posting to the list, but they should
take care of discouraging moderator-dependency. On the other
hand, they should not condescend in attending to "low-level",
"basic-text-book" questions, but serve as "facilitators" in
list activation. Every moderator has his or her own style.
Would we want look-alikes molded by regulation and by
selective membership criticisms? It is the list coordinators
that add spice to the forum and food-for-thought should not
taste all the same.


Why do we need guidelines? Why do we need to be "orderly"? The
common denominator may be complying to ethical rules,
disseminating information, defending democratic principles,
and the almost-too-trite-to-state mission of simply helping.
The Internet is full of possibilities and we are in danger of
reducing those to a minimum or, more probably, causing
more scatter, split-ups, new lists and heated, never-ending
controversy. This is exactly the situation we face everyday in
the "real world"; while it may work well for some, realize
that it also stifles others. So, if you want to carry
hierarchical real-world structures into the Internet,
you might just as well exterminate its other potentials. Why
not? After all, it's useful enough with its virtual-fax
possibilities, and the "other possibilities" are nothing but
noise - never signal. We wouldn't want to be puristically
democratic and indulge in free-floating generation of ideas,
brainstorming or naive inquiries from the not-know-it-alls.
So: Do kill me - I'm still useful as is!

                      1. InterPsych to move to Netcom
==============================================================

Sometime between December 23rd and 31st, InterPsych's mailing
lists will be relocated from Mailbase, InterPsych's current
sponsor in England, to Netcom, a commercial Internet access
provider.  InterPsych's lists at Helsinki will also be moved to
the same location.  This move is viewed as an interim step
while InterPsych sets up its own permanent location.

This move was caused by the announcement from Mailbase early
this fall that they were withdrawing their support at the end
of the year.  Mailbase's rationale for doing so was that the
majority of members on InterPsych's lists were not from the UK.
In particular, Mailbase officials noted that the majority of
list members were from the United States, putting what they
deemed as an unnecessary stress on the trans-atlantic computer
link.

The move should be "virtually transparent" for list members,
Ben Goldhagen, InterPsych's Executive Director states.  All
mailing addresses will be transferred directly over to Netcom
so that members will not have to resubscribe to groups they are
currently on.  The only significant change will be in the
address that messages are sent to.  An announcement providing
this address will be made on the superlist once the move is
complete.

One possibility is that some members names might not be
properly transferred.  In the event any member finds their
mailing list uncharacteristically silent around the time of the
move, he or she might want to try re-subscribing to the group.
Such problems however are expected to be minimal.

If all else fails, mail sent to interpsych@netcom.com will get
a response from InterPsych's maillist team.

While the announcement that Mailbase was withdrawing its
support was originally met with panic and pronouncements that
"InterPsych would cease to exist", this change has turned out
to be to the great benefit of InterPsych and its many members.
Goldhagen states that the move from Mailbase has caused
InterPsych to become more organized in its thinking, leading to
the creation of many new lists and services once the move is
complete.

InterPsych had the option to be sponsored by many different
universities and organizations.  The decision to move to
Netcom, a commercial provider, was based on the organization's
desire to maintain independence and autonomy.

Messages sent to the Mailbase address will continue to be
forwarded for 2 months after the move.  List members should,
however, attempt to send messages to the new address
immediately after the change. [SPS]

                      2. Rorschach conference report
==============================================================

The InterPsych/PsyComNet conference held on Dec. 4, 1994 was
entitled, "The validity of Dr. Rorschach's Test" which was
prompted by a discussion that began 2 weeks prior in one
of InterPsych's groups.  Unfortunately for participants,
attendance was one of the smallest for these conferences,
with only between 4 and 6 people on-line at a time. Dr. Ivan
Goldberg, M.D., was the moderator.

Dr. Goldberg began the conference with the same question
that he posed in the original discussion, namely to see the
sensitivity and specificity statistics of the Rorschach
system.  Some confusion regarding these statistics may have
been due to some participants being unaware that, in general,
the Rorschach is no longer being used to predict individuals'
behaviors or to diagnosis mental disorders, especially using
Exner's Comprehensive System, which was discussed.

It appeared that conference participants were generally
hesitant to concede that some studies (independent of Exner's
own citation of research in both Volume 1 & 2 of his
Comprehensive System) have been performed that verify the
Rorschach, using Exner's system only, can be a reliable and
valid measure.  (See, for example, two meta-analytic studies:
Parker, 1983 and Parker, Hanson, & Hunsley, 1988).  Discussion
ensued over whether other professions, such as psychiatry,
were in any better shape with regards to studies showing that,
for instance, medications are prescribed on the basis
of specific medical tests or not.

The usefulness of the Rorschach was also discussed, with some
ambivalence as to what type of additional information it could
provide.  It was noted that the Rorschach is probably not a
good predictor of treatment responsiveness, and a discussion
ensued over what could best predict responsiveness.  It was
also noted that the Rorschach is rarely used alone and is
perhaps most useful when a part of a larger psychological
assessment battery to confirm or disconfirm other tests'
hypotheses about a patient. [JG]

            3. ANOTHER ONE ON THE MOVE: EVALUATION CENTER LISTS
==============================================================

The lists of the Human Services Research Institute have
recently been moved from world.std.com to sjuvm.stjohns.edu.
They are up and running. Current subscribers will already have
received or shortly receive an acknowledgement on subscription
to the new site and software version. They will be "moved"
automatically, i.e. there is no need to unsubscribe-
resubscribe. Normal mail traffic on the lists can continue
after management's acknowledgement, around December 14.
Commands to the listserver will change, as the software has
been changed from majordomo to listserv. A special improvement
may be the format of the digest option. Old subscribers will
be sent notice of changes. New subscribers will get full
documentation once they subscribe. Address questions to Sharon

                        4. DSM IV Conference Report
==============================================================

After a series of e-mail messages regarding the new Diagnostic
and Statistical Manual (DSM IV, APA, 1994) that sounded more
like MTV's Beavis and Butthead ("DSM IV sucks") than scholarly
discussion, Ivan Goldberg held an electronic conference on
November 20, 1994 to discuss what was at the root of this
sentiment and to discuss "Medical and other models of
psychopathology".

Approximately fourteen attendees spent two and a half hours
discussing the merits of the DSM-IV as well as the merits of
diagnosis and diagnostic models in mental disorder.  Ivan
supplied for pre-conference perusal, an excerpted transcript
of the previous several weeks mail lists discussions, and it
proved useful to the conferees.  The following are a sample:

"I must disagree with the above.  The medical model,
the biopsychosocial model, of neurobehavioral disorders seems
applicable to well over "five percent" of people seeking help
from mental health professionals.  Among the disorders to
which the medical model seems especially applicable are:
ADD / ADHD;Amnesic Disorders;Bipolar Disorder;Bulimia nervosa;
Cyclothymia;Delirium;Dementia;Dysthymia;Generalized anxiety
disorder; Major depressive disorder;Obsessive-compulsive
disorder;Panic Disorder; Post concussion syndrome;
Schizophrenia; etc, etc. Why do I include these disorders?
Because they resemble medical disorders such as multiple
sclerosis, or a fractured skull, or rheumatoid arthritis, or
peptic ulceration or tuberculosis, at the "molecular, genetic,
biological, psychosocial, or sociocultural"
levels of observation."

Or:

"The DSM IV (or DSM III, DSM IIIR, DSM-anything) has little,
if anything, to do with science. It has to do with
bureaucracy, government requirements and insurance
requirements. In that respect, it takes on an entirely
different relevance. One could sit at the word-processor and
come up with something =much= simpler to satisfy the number-
crunchers and pencil-pushers."

The discussion quickly began by focusing on the politics of
diagnosis.  The APA's financial stake in publishing the DSM-IV
manual, the impact of third party payments and justification
for same, as well as "turfing" issues between professionals
were raised as possible determining factors in the DSM-IV's
development and implementation.  Ivan Goldberg's attempt to
stop the publication of the DSM-IV was pointed out and Ivan
reported on his "petition drive" to force the American
Psychiatric Association to hold a referendum on the
publication of the DSM-IV.  According to Ivan, "...we
lost...but many of the folks who voted against us now say they
are sorry."  Later in the conference, Ivan noted that, "I did
not start the drive to block its publication because of any
specific dissatisfaction with its structure... the chief
objection was that the USA and the rest of the world should
use the same system (referring to the ICD-10, and noting that
it would not be the case until an integration of both systems
was completed).

Discussion moved to psychiatric models in general and the role
of descriptive versus explanatory models quickly gained the
floor.  Is the DSM-IV merely a descriptive categorization of
behavior(s) or does it have greater utility.  Some argued that
while it may essentially be only descriptive, it offers a
common language for discussing clinical issues.  In addition,
"the DSM-IV gives us a starting point to investigate
causality", according to one conferee.  Discussion also dealt
with the use of diagnosis in the clinical process, and whether
diagnosis artificially even speciously informs the therapist
patient relationship.  Repeatedly, the influence of non-
scientific factors were pointed out.  The increasing influence
of insurance companies, and managed care entities in demanding
more "efficient" treatments coupled with more specific
diagnosis was debated.

Though often diametrically opposite, almost all views suggest
dissatisfication with the present DSM-IV system, yet few
alternatives were discussed.

conferences, IPN has set up its own subscription list.  The

6.   SZASZ "PSYCHIATRIC NEWS" ARTICLE AND IP DISCUSSION LOG
     AVAILABLE
==============================================================

Starting December 3rd, 1994, InterPsych experienced one of the
liveliest discussions ever experienced on the Psychiatry forum,
with the lists Forensic-psychiatry and Psych-current issues
also involved.  The discussion was triggered by a Psychiatric
News article concerning a lawsuit against Thomas Szasz ("Szasz
settles suit for $650,000", Psychiatric News, Nov. 18, 1994).

Dr. Jeffry Schaler offered mailing of two files - the original
Psychatric News article and the log of the discussion - to
members of the discussion forum.

Due to an unexpectedly high number of requests, he had
to withdraw his offer and the InterPsych Newsletter has
volunteered to prepare the files for automatic retrieval.

These files are now stored on the InterPsych Newsletter
listserver and may be retrieved in the following fashion:

   7. CHARLES FIGLEY RECEIVES PIONEER AWARD: NOVEMBER 1994
==============================================================

At the annual International Society for Traumatic Stress
Studies Conference in Chicago, Dr. Charles Figley was presented
with the society's highest honour, the Pioneer Award.  This
award is given to honour lifetime achievement in the field of
traumatic stress.

Dr. Figley was the founding president of the International
Society of Traumatic Stress Studies. His role in the
development of the field of traumatic stress research and
practice is well recognized.  His books and articles are
required reading for those wishing to study and specialise in
traumatic stress.  His work has covered a variety of subject
areas including the effects of Post-Traumatic Stress Disorder
on the family, family therapy and integrated approaches to
PTSD, and most recently, he has been developing the concept of
Compassion Fatigue to describe the reactions experienced by
mental health practitioners who work with traumatized people.

Dr. Figley is a professor at Florida State University and is
Director of the Psychosocial Stress Research Program.  He is
listowner of IP's traumatic-stress list and a member of the IP
board of directors.  His most recent research project is called
the Active Ingredient Series which is examining four methods of
treatment for Post-Traumatic Stress Disorder to discover what
parts of the treatments work to help sufferers recover.

                          1. INTERPSYCH UPDATE

In response to Mailbase's withdrawn support, InterPsych has
established an account at Netcom (SEE NEWS, SECTION B1) and
is in the process of transferring its mailing lists there.
Nancy Tice is coordinating the effort. The transfer will be
complete before Mailbase's December 31 deadline.

The Traumatic Stress Research Group was awarded a development
grant to be used in the furthering of the Rural-Care e-
conference.  This seed money, combined with the .25 release
time provided to Beth Hudnall Stamm, Ph.D. by the College of
Arts and Sciences of UAA will allow the group to purchase
minimal equipment and hire two part-time graduate assistant
with which to prepare applications for larger grants for the
sustanance and expansion of the Rural-Care program.

InterPsych's new groups seem to be attracting a good deal of
interest based on the following membership numbers.

                 2. SPECIAL INTEREST GROUP (SIG) UPDATE

A.   CLINICAL-PSYCHOLOGY
     -------------------
The clinical-psychology list has continued to remain an active
forum for interchange on topics relevant to the discipline.
The move from Mailbase to NetCom, however, will bring with it
a significant change in the list as well.  Presently the
clinical-psychology list is open to anyone, though it was
originally founded as a list for members and affiliates of APA

Division 12.  In keeping with the original intent of the list,
as well as the need for discussion geared towards mental
health professionals, it was decided that the move to NetCom
would bring with it membership criteria.  In line with other
IP lists such as psycho-therapy and psycho-pharmacology, the
clinical-psychology list will be open to all clinical
psychologists (who have obtained a terminal degree or its
equivalent in the field), graduate students in clinical
psychology (as well as counseling psychology), psychiatrists,
non-psychiatric physicians with related interests, master's
level social workers and above, and others whose
accomplishments or credentials are not specific to the above,
but who have a demonstratable appropriate background in these
and related areas. [JJP]

B.   MANAGED-BEHAVIOURAL-HEALTHCARE
     ------------------------------
The list, "managed-behavioural-healthcare", is sponsored by
the Institute for Behavioral Healthcare; the list-owner is
Greg Alter, Ph.D., who may be reached directly at
alter@ix.netcom.com. Currently, the list is unmoderated and
open.

This list provides an opportunity for the international
discussion of ideas encompassing the area of managed
behavioural healthcare, including, but not limited to, the
impact of health care reform in the U.S. and comparison with
other systems of behavioural health care around the world.

The list also provides an opportunity for communication among
members of the Institute for Behavioral HealthCare (Tiburon,
CA., U.S.A.), and its component organizations, including the
Council of Behavioral Group Practices and the National
Leadership Council. Contributed by Gregory Alter.

C.   PSYCH-COUNS
     -----------
PSYCH-COUNS is in the process of becoming a part on
InterPysch, but possibly won't switch over to netcom until
early January, as it isn't on the mailbase "hit-list".
PSYCH-COUNS is a maillist based discussion group for people
who are interested in theoretical or research issues in
counselling and psychotherapy. PSYCH-COUNS began in November
1994 and has, so far, been a focussed, relatively quiet
maillist. It is an open, unmoderated list. The owner is Peter
Booth.  Contributed by Peter Booth.

D.   TRANSCULTURAL-PSYCHOLOGY
     ------------------------
TRANSCULTURAL-PSYCHOLOGY has thus far been a scholarly
discussion forum of mental health service delivery to diverse
cultures. Invited topics include, cultural differences in
views on mental disorders, culture-specific syndromes,
collaboration between Western and traditional healers, and
cultural variance in symptoms. However, current and past
discussions have gone well beyond these issues, encompassing a
wide range of debates and inquiries.  The discussion highlight
has been on somatized depression, started by a member who's
Indian patient showed this symptomatology; issues of nosology,
theory, and treatment were discussed. The discussion faded
away with a short exchange on possible stereotyping (i.e.
somatizing Asians and psychologizing Westerners). There were
various comments to a posting on indigeous (Maori) women and
body image. Inquiries were on acculturation scales for Puerto
Ricans, attributional styles, cross-cultural aspects of
alcohol treatment. Consensus in the forum was reached to put
forth announcements of calls for papers, conferences and other
news, general and related to transcultural-psychology to the
list; input in this regard is welcome from all members and
non-members. A number of old and new members introduced
themselves, illustrating again that the background of the
subscribers - psychology in research and practice, literature,
social work, different minority group members and nations - is
quite diverse. Beginning in 1995, probably Feb./March, there
will be some changes in the discussion forum, expanding from
mental health service delivery to broader topics in
cross-cultural psychology and minority issues. Everybody is
welcome to help in the facelift, either by subscription to the
list and active participation in the discussion, and/or by

A.   ADDICTION MEDICINE
     ------------------
This conference has been set up to encourage an exchange of
ideas, opinions, and information among health care
professionals working in the field of addiction to alcohol
and/or other psychoactive substances. A broad working
definition of addiction follows: "Addiction is a disease
characterized by continuous or periodic; impaired control
over the use of drugs or alcohol, preoccupation with drugs or
alcohol, continued use of these chemicals despite adverse
consequences related to their use, and distortions in
thinking, most notably denial." (Adapted from the American
Society of Addiction Medicine).

We recognize that there is a spectrum of use of alcohol and
some psychoactive drugs, from intermittent use that does not
interfere with health, to malignant destructive use. For the
purposes of this list we would in general encourage open
discussion about all aspects of addiction as defined above.

Please note that this list is intended for those working in
the field and is not to be used as a recovery list to work on
individual issues. Such work is supported by many lists on
Internet. Strong opinions are welcomed, but flaming will NOT
be tolerated!

B.   CHILD AND ADOLESCENT ANXIETY AND DEPRESSION

This special interest group has been set up to encourage an
exchange of ideas, opinions, and information amongst those
involved in research and clinical practice in the areas of
child and adolescent anxiety and depression. The group is an
interdisciplinary one involving a variety of mental health
professions.

The group will take a scientist-practitioner approach to the
nature, etiology, assessment, and treatment of anxiety and
depression problems amongst children and adolescents. It is
hoped that the network will serve several functions such as:
*    drawing attention of other members to recent relevant
     publications in the field
*    summarizing one's own research and recent findings that
     are likely to be of interest to others (It is planned to
     invite contributions from group members on an occasional
     basis)
*    requesting information, comment or advice from others re:
     relevant topics
*    discussion of new assessment or treatment approaches

The group is NOT a forum for on line diagnostic advice. Nor is
it a forum for airing your own subjective personal views that
are unsupported by empirical evidence or strong theoretical
bases. Given that the group is an interdisciplinary one it
should not focus excessively upon medication issues. To join

A new electronic conference, geriatric-neuropsychiatry, has
been started as part of InterPsych, the international
consortium of resources in psychiatry and abnormal psychology.
This conference has been set up to encourage an exchange of
ideas, opinions, and information among persons concerned with
neurobehavioral disorders in middle-aged and elderly adults.
For purposes of this discussion, 'neurobehavioral disorders'
are defined as medical or neurologic disorders leading to
changes in cognition, mood, perception or behavior. Although
this list is primarily
intended for clinicians, input from caregivers, basic
scientists and other concerned individuals is welcomed.

Introduction

Sleep apnea can cause symptoms of apparent clinical depression
which may respond quickly to treatment of the sleep disorder.
A knowledge of the intellectual and emotional impact of sleep
apnea may be important in making a correct diagnosis.
Unfortunately, many physicians, psychiatrists, and
psychologists may be unaware of this connection. Yet these
professions have much to contribute to dealing with this
problem and informed individuals can provide essential therapy
and support to the patient and the patient's family in the
recovery process.

The impact of sleep apnea and snoring on sleep and health in
the individual and for the public should not be ignored.
According the National Commission on Sleep Disorders Research:
"Forty million Americans are chronically ill with various
sleep disorders...Sleep apnea alone is the cause of excessive
daytime sleepiness experienced by almost 20 million
Americans... Overwhelming evidence from testimony and specific
surveys suggests that the vast majority of Americans with
sleep disorders remain undiagnosed and untreated. ... "
(Report of the National Commission on Sleep Disorders
Research, "Wake up America: A National Sleep Alert," Executive
Summary and Report, January 1993.)(hereafter cited as NCSDR)

What is sleep apnea?

"Obstructive sleep apnea is an illness characterized by
snoring, partial or complete cessation of breathing during
sleep, reductions in blood oxygen levels, severe sleep
fragmentation, and excessive daytime sleepiness. Researchers
and clinicians have recognized sleep apnea as one of the most
common sleep disorders and with perhaps the greatest medical
and social impact on society in terms of morbidity and
mortality. The syndrome strikes both sexes and all races,
ages, socioeconomic strata, and ethnic groups, though it is
less common in women prior to menopause, and may be more
common in blacks than in whites." (NCSDR)

"The potential consequences of obstructive sleep apnea are
significant and include hypertension, coronary heart disease,
myocardial infarction, pulmonary hypertension, congestive hear
failure, stroke, neuropsychiatric problems, cognitive
impairment, sexual disfunction, and injury due to accidents.
Finally the Commission estimates that cardiovascular deaths
attributable to obstructive sleep apnea alone may be as high
as 38,000 annually." (NCSDR)

"In the United States, 11,845,000 people between the ages of
30-60 are estimated to have obstructive sleep apnea; nearly
one-quarter of them (3,029,000) experience the disorder at a
moderate or severe level. The prevalence of obstructive sleep
apnea may be much higher in specific subgroups. ... For the
approximately 31 million Americans who are age 65 and older,
the best estimate of obstructive sleep apnea is 7,440,000,
with 46 percent at the moderate or severe level." (NCSDR)

Recent studies show that among employed people 30 to 60 years
old, at least 2 percent of the women and 4 percent of the men
may have treatable sleep apnea syndrome, while 24 percent of
the men and 9 percent of the women showed disturbed breathing
during sleep. (Terry Young et. al. in the New England Journal
of Medicine, April 1993). An editorial by Eliot Phillipson, MD
in the same issue of NEJM proclaimed sleep apnea as "a major
public health problem."

Treatment

Effective treatment eliminates snoring and apnea. The success
of treatment is measured by the reduction of respiratory
disturbance to normal levels, by the elimination of symptoms
like fatigue and depression, and the patient's subjective
feeling of well-being. The "gold standard" treatment accepted
by physicians and others trained in sleep disorders medicine
is the continuous application of positive air pressure through
a mask during sleep (Continuous Positive Airway
Pressure--CPAP). The additional air pressure keeps the airway
from collapsing. Since this permits normal breathing to
continue during sleep, normal sleep patterns emerge, sleep
becomes restorative, and the patient feels better. The impact
is often immediate and dramatic although the patient may need
to continue therapy permanently. Weight loss, surgery and
other interventions may also have their place in treatment.
Diagnosis is made on the basis of the clinical picture,
patient and spouse reports, and an overnight polysomonogram
done in the laboratory or even at home.

The role of psychologists and psychiatrists

Even a basic understanding of sleep apnea can be the basis for
awareness and referral. A patient presenting with fatigue,
depression, or problems in social relationships might have a
primary problem of sleep apnea. If so, treatments appropriate
for depression may miss the mark. If snoring, gasps, or pauses
in breathing during sleep are reported, referral to an expert
in sleep disorders for evaluation may be appropriate.

Psychologists and psychiatrists can play an important role in
the treatment of sleep apnea. With appropriate training in
sleep disorders, they can play a part in the sleep laboratory
or in the clinic.

The patient with sleep apnea faces tremendous social and
emotional challenges in accepting therapy. A significant
number of patients fail to comply with treatment, sometimes
because they may lack appropriate emotional support and are
unable to adjust to their new status. Family members and
colleagues may not understand or accept the positive changes
in the patient's personality. And the patient and those close
to him or her must come to grips with the loss of functioning
for years before treatment. Finally, if symptoms of depression
occur in a patient with known sleep apnea, appropriate
treatment may require looking at both the treatment for sleep
apnea--is it effective?--and at the possibility of depression.
While there are support groups and patient-centered
organizations which can be very important to patients and
their families, professional intervention and support may also
be needed.

The Evaluation Center@HSRI (TEC) is a technical assistance
center funded by the federal Substance Abuse and Mental
Health Services Administration & Center for Mental Health
Services.

The mission of the Evaluation Center@HSRI is to provide
technical assistance in the area of evaluation to States and
nonprofit public entities within the States for improving the
planning, development, and operation of adult mental health
services.

Following is a brief description for each Topical Evaluation
Network (TEN). Although this information could have been
placed in the resources section of the InterPsych Newsletter,
we would like to inform readers that there is a sizable number
of subscribers, particularly on the evaluation and outcome
lists, that are currently running research projects, which
may lend themselves for matching up researchers with
similar interests.

     The purpose of this network is to discuss how national
     health care reforms may affect mental health care and
     substance abuse systems at the state and local levels.

A group at the University of Rochester has created a
home page on the World Wide Web (WWW) for the two
SEAB journals, Journal of the Experimental Analysis of
Behavior (JEAB) and the Journal of Applied Behavior
Analysis (JABA). This effort is nothing short of
spectacular.

This "page" is still somewhat in progress but there is
already a complete and searchable index of JEAB and a
partial sampling  from JABA. This index can be
searched using key words provided or of your own
making. Logical arrangements of key words (and, or)
can be made. You retrieve complete reference =and=
abstract. The index searches titles and abstracts. This
index, I'm told, will be kept up to date with future issues.
In addition you can read historical information about the
society and the journals. Pictures may follow in the
future.

In the posting I forwared, instructions were given on how
to access it in different ways. If you have MOSAIC then
you can enter a URL after clicking on the appropriate

The CLIMATE Clinical Help System is a Hypertext mental health
information system based on the World Health Organisation
(WHO) Primary Health Care Version of ICD-10, augmented with
information from the  CRUfAD Community Mental Health Workbook.
The information contained in  this Clinical Help System is not
a prescription; rather, it is advicethat clinicians may use
to inform themselves about assessment strategies, diagnostic
issues, and management for individual patients. Responsibility
for clinical decision making remains with the clinician. The
system is exactly what its name implies easily accessible
clinical help for the busy clinician. There is a good deal of
commonly used information in this system and future releases
of CLIMATE will include further information and enhancements
to its capabilities.

           2.Call for collaborators: EEG, ERP, Sleep studies

The IBEGROUP (Internet based Biomedical Engineering GROUP)
invites experts from around the world to participate in the
development of a state-of-the-art biomedical data acquisition
system for EEG, ERP and SLEEP studies .

The IBEGROUP is a non-profit Internet based distributed
product development group. The current project involves the
development of a high performance 32/128 channel biomedical
data acquisition system.
   The group's objectives are:
   (1)    Make a contribution to the science of biomedical
          instrumentation
   (2)    Provide biomedical engineers with an opportunity to
          develop interdisciplinary contacts worldwide,
   (3)    Introduce participants to the art of product
          development, from conception to manufacturing.
   (4)    Produce a superior data acquisition system for
          educational and research programs.

Funding will be sought through grants, sponsors, and
support from users and manufacturers of biomedical equipment.
The group seeks expertise in such areas as: low voltage, low
power, ultra low noise, fiber-optics, telemetry, analog and
digital IC design, power/signal isolation, DSP, computer
interfacing, and manufacturing.

Scientists, engineers, technologists and educators with
relevant experience, as well as users and manufacturers of
biomedical equipment, are invited to participate in this

                      3. Outpatient certification
=============================================================

I am interested in the issue of out-patient certification as
an alternative to compulsory in-patient treatment, or as a
vehicle to give authorisation for treatment of mentally-ill
individuals who are deteriorating and have no insight into
their own illness.

There are approximately 26 states in the USA which have such
legislation, and I am interested in hearing from physicians
who have been involved in the drafting, or implementation, of
these laws. Comments from physicians who support, or have
criticism of, this legislation, would also be appreciated. I
would also appreciate the comments of Psychiatrists or
Family Physicians from other jurisdictions who have
experience or advice.

The Ontario College of Family Physicians, a 5,000 member
organisation, is interested in lobbying for such legislation
in Canada. Out patient certification has recently been
recommended by several coroners' juries investigating the
deaths of schizophrenic patients in our province, the latest
being the death of Lester Donaldson, a schizophrenic who was
shot by police in a violent confrontation in Toronto.

Comments on issues such as the criteria for out-patient vs
in-patient certification, who would be the decision-maker
(physician or patient designate), and the success or
otherwise of such interventions, would be appreciated.

In February, 1994, Dr. Michael Cox and I (Dr. Gary E.
Jones) presented the first U.S.M. - Baylor College of
Mecidicine WINTER SYMPOSIUM ON APPLIED PSYCHOLOGY at CRESTED
BUTTE, COLORADO. The first year theme was clinical
psychodiagnostic assessment and presenters/faculty included
Dr. Les Morey (P.A.I.), Dr. Robert Archer (MMPI-A), Dr. Lynn
Hungerford (Rorschach Test), Dr. Mike Cox (Forensic
Evaluations), and Dr. Dave Lachar (PIC). The theme of the
Symposium is intended to change for each
offering.

The Symposium was geared to provide a first-rate
continuing education experience for applied psychologists who
need to do periodic continuing education in a setting
conducive to winter sports activity or winter
mountain enjoyment

qualifications needed:

The main themes for this professorship are psychometrics,
data-analysis and research methods within the field of
psychology. Candidates must have comprehensive theoretical
competence as well as practical research competence within
evaluation of candidates.

Duties for the position will be divided between research,
teaching and administration. Currently, teaching and
research will account for about 47% each, and administration
for 6% of the time. Teaching will include about 4 hours of
lecturing per week as well as participation in the
supervision and evaluation of student work at different
levels of study.  For all tenured positions there is a
required language proficiency test within two years time for
non-Scandinavian English-speaking candidates.

The salary for full professors is currently
about NOK 300,000($40,000).

FULL-TIME DEPUTY COURSE DIRECTOR
TRAINING COURSE IN CLINICAL PSYCHOLOGY (DOCTORAL DEGREE)

The Wessex Regional Training Course in Clinical Psychology
is now part of the Department of Psychology at the
University of Southampton, and will move to the
Highfield campus in 1995.  The Course, which now offers a
Doctorate in Clinical Psychology, will have an annual
intake of nine trainees from 1995.

The Deputy Director post requires an experienced
Chartered Clinical Psychologist to work closely with the
Director, Dr Maggie Cormack.  The main responsibilities
include the development, planning, organisation, and
monitoring of clinical placements; teaching, research,
and research supervision. The postholder will work closely
with the members of the Course team, healthcare providers,
local district, unit, trust and speciality heads of
service, and  current and potential supervisors.

and physical health outcomes of rape victims seen in

The NCVC fellowship training program is designed to
produce professionals skilled in using a variety of
research strategies to investigate the psychosocial impact
of traumatic events upon adults and children and the
effects of intervention.  The program includes advanced
clinical training in cognitive-behavioral and family-based
therapy procedures and techniques.  Advanced research
training focuses on sexual assault, other violent crimes,
natural disasters, urban violence, and other forms of
civilian trauma.  The prevention, development, and
treatment of Post-traumatic Stress Disorder, other anxiety
disorders, Major Depression, substance abuse, and other
post-trauma adaptations are all topics of the fellowship.
Fellowship activities include a strong preceptor system,
ongoing training seminars, supervised clinical intervention
with adults, children, and families, participation in
existing funded research projects and the development of
new research projects.  The fellowship is open to entry
level and experienced professionals.  The fellowship is for
one year with an opportunity for a second year renewal.

scientist-practitioner model.  Applicants should hold a
doctoral degree in psychology, social work, medicine, or
related field.  Stipend levels are contingent upon

Due to problems related to the move to Netcom, release of the
January and February issues of the InterPsych Newsletter was
cancelled. We regret any inconvenience that this has caused.
Starting this month, the newsletter will be back on a regular

The Brain Behavior Laboratory and the MHCRC on Regional
Brain Function in Schizophrenia at the Departments of
Psychiatry, Neurology and Radiology,  University of
Pennsylvania School of Medicine, have two positions for
Postdoctoral training in Neuropsychology and related
neuroscience fields. This is a Federally-funded training
program with the main focus on neuroimaging applications
to understanding behavior and its aberration in brain
disorders. The Fellowship is renewable for up to 2 years,
and is aimed at preparing matriculants to independent
careers in neuroscience research. The program includes
dydactic work and direect research and clinical experience,
if appropriate, commensurate with trainee's background and
interests. Stipend is determined by NIH guidelines based
